Here's some back surface anchor piercings. I pierce these with a needle, so no tissue is removed during the process. There are many ways of doing dermal piercings, but unfortunately, piercing these with a biopsy punch (dermal punch) is illegal in the United States. I also find there is less pain and bleeding when surface anchors are done with a needle.
